Introduction

This Alpha Arizona Name Change Kit is a complete do-it-yourself name change kit designed especially for the non-lawyer.

This kit includes predrafted legal forms which provide for a legal change of name under each of three situations, namely: (1) An adult person, age 18 or older, (2) a parent or guardian on behalf of a minor child, age 17 or younger, and (3) an entire family including the parents and minor children.

If the reason for purchasing this kit is for the change of name of an entire family, keep in mind that the Joint Petition for Change of Name (Forms 300a-b) can only be used if the action will include one or more parents and one or more minor children, or two adults without minor children.

Every effort has been made to simplify to the understanding of a non-lawyer, both the statutory and procedural requirements necessary for accomplishing a legal change of name under current Arizona Laws.
